Building community health care from the ground up

The Gabriola Community Health Centre is a true success story of a community coming together to wrap care around local patients. Gabriola Islanders banded together to identify gaps in care, build new clinic space to house doctors and other health care services, and then staff the clinic with a team of health care professionals who work together to ensure that Gabriolans have seamless access to primary and secondary care without needing to leave the island.

Two interlinking stories detail the grassroots community effort to plan build the health centre, and highlight the success of the team-based care model now functioning within the clinic.
