The GPSC PMH Evaluation Framework has been distributed to all divisions. It will measure progress toward implementing the patient medical home (PMH) model at the provincial level, and system changes resulting from adopting the model. Additional information and materials will be shared with divisions as the process unfolds.

In addition to the GPSC PMH Evaluation Framework, the Divisions Impact Measurement Framework (IMF) is underway. These complementary frameworks help to tell an integrated story about the impact of divisions, physicians, and the GPSC on the PMH model, and how physician leadership, as achieved through divisions, is fundamental to improving primary care. Note: Some family doctors working at hospitals may also be involved in Facility Engagement Initiative evaluation. It is a separate evaluation and not connected to these two frameworks.
