Family physician longitudinal care payments coming in summer 2020

Eligible family physicians will soon receive the first installment  of this year’s Community Longitudinal Family Physician (CLFP) Payment if they’ve already submitted the GPSC Portal Code (14070). GPSC has decided to make the 2020 CLFP Payment in two installments and has fast-tracked the first installment.

The first installment of $3000 will be paid in summer 2020. The second installment will be paid in winter 2020, and it will be based on the number and complexity of patients associated with the family physician.

The combined total of the two installments will be between $3000 to $12,000 per eligible family physician. Most physicians will receive between $4000 to $8000 in total for their 2020 payment.

The GPSC Billing team at GPSC.billing@doctorsofbc.ca will email all eligible doctors a notification of their payment amount by June 30, 2020. GPSC Billing will also communicate the payment date once it’s confirmed by the Medical Services Plan (MSP).

The CLFP Payment recognizes community-based family physicians who provide relationship-based care to a panel of  patients over time. In its ongoing support of  this payment, the GPSC  has taken further steps in supporting the vital contribution of longitudinal care to better outcomes for patients and health systems.  

Eligible family physicians will receive the 2020 CLFP Payment if they:

  • Submit and meet the requirements for the GPSC Portal Code (14070)  by July 3, 2020.
  • Show in billing data to be providing longitudinal care to a group of patients through the MSP database.

 When a physician submits the GPSC Portal Code (14070) it signifies that they are: :

  • Providing full-service family practice services to their patients, and will continue to do so for the duration of that calendar year.
  • Confirming their doctor-patient relationship with their  patients through a standardized conversation or the “family physician – patient compact”.

Eligible physicians who submit the GPSC Portal Code (14070) after the summer payment is made will receive one lump sum for their total payment in fall 2020.

The GPSC has allocated funding to ensure new to-practice physicians who are not yet eligible for the payment based on majority source of care (MSOC) patients may receive the minimum CLFP payment of $3,000 during their first year of practice. GPSC will notify eligible family physicians by email in winter 2020.

Who is eligible?

Fee-for-service, community-based family physicians are eligible for the 2020 CLFP Payment if they meet the following conditions:

  • Meet the requirements for and submit the GPSC Portal Code (14070) by the MSP cut-off date of Friday, July 3, 2020.
  • Had 50 or more patients for whom the physician was the Majority Source of Care (MSOC) provider based on fee-for-service family physician visits in 2019.

 
How much is the payment and when will it be paid?

  • The first installment of $3000 will be paid in summer 2020 to eligible family physicians.
  • The second installment, a variable amount up to $9000 that’s tied to the number and complexity of Majority Source of Care (MSOC) patients associated with the family physician, will be paid in winter 2020.
  • The combined total of the two payment installments will be between $3000 to $12,000 per eligible family physician, with the majority of eligible family physicians receiving $4000 to $8000.
  • Eligible physicians who submit the GPSC Portal Code (14070) after the summer payment is made and before the July 3, 2020 MSP deadline will receive one lump sum for their total payment in winter 2020.

How will payment be made?

  • MSP Teleplan will automatically remit payment to eligible family physicians. The payment will be identified by adjustment code “CP” in the physician’s remittance statement.
  • The 2020 CLFP Payment is paid to the payee number where the physician provided the majority of family physician services in 2020.
  • Payment will be made in two installments. The first installment is $3000 paid in summer 2020 to eligible family physicians. The second installment, paid in winter 2020, is a variable amount up to $9000 based the number and complexity of Majority Source of Care (MSOC) patients associated with family physician in MSP database.
  • Eligible family physicians who submit the GPSC Portal Code (14070) after the first installment is paid in summer 2020 and before the MSP cut-off date of July 3, 2020 will receive one lump sum payment for their full amount in winter 2020.
  • Prior to MSP payment, each eligible family physician will receive an email from GPSC.billing@doctorsofbc.ca with information about their specific payment.

 

What is the deadline for submitting GPSC Portal Code (14070)?

  • Family physicians must meet the requirements for and submit the GPSC Portal Code (14070) by the MSP cut-off date of July 3, 2020 to be considered for the 2020 CLFP Payment.
  • Family physicians who submit the GPSC Portal Code (14070) by the MSP cut-off date of July 3, 2020 must also have been the Majority Source of Care (MSOC) provider for 50 or more patients based on fee-for-service family physician visits in 2019 to qualify for the 2020 CLFP Payment.

 

What if I submit the GPSC Portal Code (14070) after the first payment in summer 2020?

  • Family physicians who submit the GPSC Portal Code (14070) after the first payment installment in summer 2020 and by the MSP cut-off date of July 3, 2020 will receive their full amount as one lump sum payment in winter 2020 if they meet all of the requirements of the 2020 CLFP Payment.

 

Do family physicians have to provide a portion of the CLFP Payment to clinic owners?

  • The 2020 CLFP Payment is paid to the payee number where the physician provided the majority of family physician services in 2020.
  • The CLFP Payment is intended to provide additional financial support to fee-for-service family physicians who provide longitudinal care to patients.
  • All GPSC incentives paid through MSP Teleplan, including the CLFP Payment, may be subject to clinic-specific business arrangements pertaining to clinic overhead costs.
  • In these cases, physicians and clinic owners are advised to come to a mutual agreement on how existing business arrangements apply to the CLFP Payment.

 

What is a Majority Source of Care (MSOC) patient?

  • MSOC patients are those who, during a 12-month period, visited a family physician three or more times with more than 50% of those visits to one family physician. A patient can be MSOC for only one family physician.
  • One or more family physician services (including telephone visits) provided by a single family physician to a single patient on a single day is considered as one patient visit to a family physician.
  • For the CLFP payment, only family physician visits provided under fee-for-service are considered.
  • Exclusion note: Consultative and surgical assistance visits as well as visits provided in facilities such as hospitals and long term care facilities are not considered.

 

Are family physicians who took parental leave in 2019 eligible?

  • In many cases, family physicians who took parental leave in 2019 will still meet the eligibility requirements of the 2020 CLFP Payment and be eligible for an annual payment amount of $3000-$12,000.
  • In some cases, family physicians may meet the requirements for and submit the GPSC Portal Code (14070) in 2020 but may not be the Majority Source of Care (MSOC) provider for 50 or more patients in 2019, due to parental leave in 2019. In these situations, physicians who were the MSOC provider for 50 or more patients in the calendar year preceding the start of parental leave will be eligible for a 2020 CLFP Payment of $3000.
  • 2020 CLFP Payments of $3000 to physicians in this situation will be paid to eligible physicians in winter 2020.

 

What’s new for 2020?

  • The fast-tracked payment schedule – Payment will be made in two installments. The first payment of $3000 will be made in summer 2020. The second payment will be paid in winter 2020 to eligible family physicians. The amount of the second payment will be determined by the number and complexity of Majority Source of Care (MSOC) patients associated with each family physician in the MSP database.
  • GPSC Portal Code (14070) deadline – To be considered for the 2020 CLFP Payment, family physicians must meet the requirements of and submit the GPSC Portal Code (14070) by the MSP cut-off date of July 3, 2020.
  • Payee Number – The 2020 CLFP Payment will be paid to the payee number where the physician provided the majority of family physician services in 2020.
