Community Longitudinal Family Physician (CLFP) Payment

Application deadline: September 30, 2022

The CLFP payment recognizes community-based family physicians who care for a panel of patients for providing long-term, relationship-based care.

Ongoing applications

Team-Based Care Grant

Application deadline: Ongoing

The new Team-Based Care Grant provides $15,000 to eligible family practices that have onboarded interprofessional team members. The grant will help address the costs of recruiting and onboarding into a practice, which physicians have identified as a barrier to creating team-based care practices.

Panel Management Incentive

Application deadline: Ongoing

In addition to sessional dollars for time spent completing the phases of panel management, GPSC offers a $3,000 bonus payment once all 3 phases are complete. Compensation is designed to support community longitudinal family practice doctors and their teams in using their EMR systems to identify and respond to their patients chronic and preventative care needs.

HDC Discover Payment

Application deadline was: March 31, 2022

Family doctors who have completed panel management are eligible for a new $1,000 GPSC payment if they enrol in HDC Discover and complete the orientation in the web based application. The payment supports family doctors to utilize the application to make data-informed quality improvements in their practices.

Group Family Practice Development Grant

Application deadline: funding is now fully committed and as such no new applications are being accepted at this time. 

The GPSC has introduced a new grant to help cover expenses incurred when family doctors join or form a new or expanded group practice. The Group Family Practice Development Grant, which provides up to $30,000 per eligible physician, addresses feedback from family doctors that significant expenditures of closing and/or merging offices are holding them back from forming or consolidating group practices.

BC Care Bundle Payment

Application deadline was: May 31, 2021

A GPSC one-time payment with a minimum of $1000 that supported eligible family doctors who took on the additional work of identifying and treating patients with care needs noted in the Ministry of Health’s BC Care Bundle for Supporting High Risk Patients Through COVID-19 Pandemic and Influenza Season.
