PMH Assessment

The PMH Assessment is an electronic self-assessment that is designed to:

  • Identify practice strengths and opportunities in relation to the 12 attributes of the patient medical home in BC.
  • Gather information that will inform and evaluate the transition to an integrated system of care at a community and provincial level.

The self-assessment takes approximately 30 minutes to complete. Family doctors will receive a short summary report immediately upon completion and a comprehensive summary via email.

Compensation is available for completing the assessment, and accreditation is available for doctors who engage with PSP to work on practice improvements based on their results from the assessment.
